For prescription renewals without an appointment, ask your pharmacy to fax a renewal request to 416 487 2508. A non-OHIP refill fee may apply if you are not on the annual plan.
Requests for new medications, specialist referrals, notes, forms or imaging require an appointment with your doctor.

Please call or email cancellation@unifyhealth.ca more than 48 hours before your appointment to cancel. Otherwise, a no-show charge will apply. We do not accept cancellations by voicemail.
