Sunnybrook Emergency Department Dermatology Referrals

This page is for patients who were seen in the Sunnybrook Emergency Department and referred to Unify Health Dermatology for outpatient follow‑up.

How to Book Your Dermatology Appointment

  1. Click the booking link here provided to you by the Sunnybrook ED team.

  2. You will be directed to our secure online booking platform (Medeo).

  3. Select an available appointment with one of our dermatologists.

Once you submit your booking request:

  • You will receive automatic reminder messages (email and/or text) before your appointment.

⚠️ Important: Your appointment will only be confirmed once we have received your referral from Sunnybrook.

Referral Confirmation Process

  • Sunnybrook ED will fax your dermatology referral to our clinic. This will be done automatically by the Sunnybrook staff.

  • Our team reviews all incoming referrals before approving appointments.

  • If we have not received your referral within 24 hours of your booking request, our staff will contact you directly to help clarify or coordinate next steps.

  • If you can provide a Sunnybrook ED contact or unit clerk name, this may help us follow up more quickly.

If a booking request is made without a referral, our team will reach out to you to avoid delays in care.

Appointment Location & Arrival Instructions

Unify Health Clinic
25 Industrial Street, Toronto, Ontario
Located above the Shoppers Drug Mart / PetSmart plaza

  • Free parking is available on site.

  • The building is elevator accessible.

  • Dermatology appointments may take place in Suite 207 or Suite 208.

  • Please follow posted clinic signage when you arrive or ask at reception if unsure.

We recommend arriving 10–15 minutes early for your first visit.

General Dermatology Visit Information

Please bring:

  • Your health card

  • A list of current medications

  • Any photos or documents provided by the Sunnybrook ED team (if applicable)

If dressings are present, please do not remove them unless instructed by the ED.

If Your Symptoms Worsen

Your Unify Health dermatology visit is for outpatient follow‑up only.

If you experience urgent or worsening symptoms, including:

  • Fever or chills

  • Chest pain

  • Shortness of breath

  • Rapidly spreading rash

  • Severe pain, bleeding, or signs of infection

Please return to the Sunnybrook Emergency Department immediately or call 911.
